Patient Educator, RNDescription: Assist patients and caregivers with educational needs, problem resolution, and health management across the continuum of care Develop educational programs for specific disease processes i.e. diabetes, stroke, CHF, etc. Collaborate with an interdisciplinary team in the planning, implementation, and delivery of educational curriculum Provide patient specific education to a wide variety of audiences Complete intake of patient consults for specific disease processes i.e. diabetes, stroke, etc. Experience/ Qualifications: Graduate of school of Professional Nursing Licensed by the Georgia Board of Nursing required BSN preferred Current BLS certification required 2+ years of clinical nursing experience required
