ESSENTIAL FUNCTIONS: Describe each major function and indicate the percentage of effort each function represents (maximum 25% for each) to total 100%. Include at least 4 or 5 essential functions (max 10)
- Develop a treatment plan for patients with specific goals and tasks
- Evaluate patients' functional ability through various standardized tests, measures patient's strength, motor development, sensory perception, functional capacity, and level of independence with daily activities, and records findings to develop treatment programs
- Records treatment, responses, and progress in the patient’s electronic medical record and prepare reports as indicated by licensure, the provider, and or insurance recommendations
- Reviews physician's referral (prescription), patient's condition, medical history, and submitted medical records to determine occupational therapy treatment required. Plans and prepares written treatment program based on evaluation of patient and incorporate proper interventions that can help patients better perform everyday tasks, fine motor coordination, and improve strength and dexterity.
- Instructs patient and family in treatment procedures to be continued at home.
- Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit
- Completes documentation per clinical / department guidelines and meets expectations of monthly department chart audits.
- Administers treatments involving application of physical agents, such as cold packs, contrast baths, moist packs, ultraviolet and infrared lamps, Iontophoresis, electrical stimulation, and ultrasound machines
- Follow facility and OSHA safety rules and procedures while on assignment
- Complete required orientation as directed by facility
- Achieves the established average monthly patient care productivity metric
- Promoted / educated Tidelands Health Rehabilitation programs to referring physicians, healthcare providers, and or co-workers by performing in-services and/or marketing activities
- Follow facility and Joint Commission guidelines and recommendations
- Proactively impacts his or her cancel / no show percentage as it relates to improving quality care and patient outcomes
- Uphold HIPAA regulations
- Punctual and dependent for assigned/confirmed shifts
- Respect cultural and religious practices of patients
REQUIRED/MINIMUM QUALIFICATIONS: Describe the minimum job requirements: formal education/training and years of experience, certificates, licenses and knowledge, skills, and abilities.
EXPERIENCE:
LICENSURE/CERTIFICATION: Licensed by the state of South Carolina Board of Occupational Therapy Examiners
KNOWLEDGE/SKILLS/ABILITIES: Ability to utilize therapy equipment a Tidelands Health rehabilitation setting to provide treatment and procedures preferred.
ADMINISTRATIVE SUPERVISION: Include number of exempt/non-exempt employees administratively supervised (authority for all personnel actions, hire, fire performance appraisal, set salary, corrective discipline).

FUNCTIONAL SUPERVISION: Include number of exempt/non-exempt employees functionally supervised (assign/review work, participate in interviews, input on performance appraisals).

PHYSICAL REQUIREMENTS: Medium-Heavy: While performing the duties of this job, the employee is frequently (activity or condition exists from 1/3 to 2/3 of the time) required to stand, sit, and walk; occasionally (activity or condition exists up to 1/3 of the time) to use hands, fingers; and frequently to talk or hear. The employee must exert 35 to 50 pounds of force occasionally, and/or 15 to 25 pounds of force frequently, and/or 10 pounds of force constantly to move objects. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
