Form Health is an innovative company delivering a multi-disciplinary weight loss intervention through telemedicine. Our mission is to provide personalized, research-based treatment delivered through modern technology. We are seeking an advanced practice clinician (NP or PA) focused on obesity medicine to provide telehealth longitudinal care for patients with obesity, and also to assist the practice with supporting patients between visits. All patients at Form Health work closely with a registered dietitian through video visits, text messaging and photo journaling, and receive customized educational materials, through our proprietary mobile app. Board certified physicians perform a virtual comprehensive evaluation and provide longitudinal care. We are now seeking to expand our team of advanced health practitioners to evaluate patients with obesity presenting for weight management, provide follow-up care between video visits, and help support patients with between-visit needs. This is a full-time opportunity; work schedules can be very flexible.

We seek applicants who have experience in medical weight management, and in telehealth delivery of care. They must have an active, unrestricted license in either CA, TX, NY, IL, MA or FL, and preference will be given to people licensed in multiple states from this list and with active credentialing in at least one of these states. Excellent interpersonal skills for patient and team interactions, commitment to the highest quality of medical care, and an evidence-based approach to obesity management are essential.
