Nurse Practitioner - Tele Pain

Duties

Duties include but not limited to:

• Follow and provide longitudinal care to veterans engaged in the telehealth program
• Maintain current credentials and licensing
• Stay current on telehealth, opioid safety, and other relevant VA and state policies
• Prescribe medications for veterans with chronic pain and opioid use disorder within their scope of practice and licensure, including buprenorphine, other opioids, and other controlled substances as appropriate
• Participate and support telehealth programs including individual and group visits
• Utilize multiple electronic information technologies including telehealth communication systems and electronic health record systems
• Maintain an "X-Waiver" with the DEA to prescribe Buprenorphine for opioid use disorder
• Perform chart reviews of consultation and referral requests, assist with care coordination and scheduling including e-consultation if needed
• Provide coverage for other providers as needed to support the telehealth team
• Develop treatment plans based on the patient's condition and diagnoses including potential referrals to other telehealth providers, recommendations for treatment and therapy including medication if appropriate
• Assess veterans via telemedicine for conditions including headache and chronic pain as well as associated conditions such as psychiatric disorders, substance disorders, comorbid medical issues and psychosocial stressors.
• Coordinate care with other telehealth providers which may include physicians, psychologists, social workers, and other staff in an interdisciplinary manner
• Respond to secure messaging and communicate with patients, outside providers and primary care providers as needed
• Provide prompt responses to patient's urgent health care needs consistent with VA guidelines
