Home Health Care

Hospice

Managed Care (2+ years)

Do you have experience with computer-based data management programs? (yes)

Are you fully vaccinated? (yes)

Do you have experience conducting annual wellness visits or similar comprehensive visits and appropriately charting HCCs; general knowledge of risk adjustment. (yes)

Are you seeking full-time? (yes)

Preferred

Valid Drug Enforcement Administration (DEA) license/certification

Palliative Care (2+ years)

dialysis patient care (2+ years)

Are you bilingual English/Spanish? (yes)

What You Need To Know

• NP panels are around 100 - 200 patients. They will be seeing 2-4 patients a day when in the field.
• This is a full-time, home health-based position with travel reimbursements provided.
• Field/Territory: To Be Assigned within 2 Hour Driving Radius

Additional Job Details

• Complete all documentation and paperwork in a timely manner.
• Maintains quality of care and productivity standards as defined by the practice.
• Prescribe medications, order tests, and collaborate with patient’s Monogram physician.
• Collaborate with care management RNs and SWs to ensure consistent, highly satisfying patient experience, accurate documentation, and effective care planning.
• Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first.
• Documents items such as: chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan.
• Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily.
• Conducts assessments, which includes comprehensive annual wellness exams on patients. (Assessments will occur in-home.)
