Mental Health Case Manager

Monday - Friday from 8 AM - 4:30 PM

No weekends or holidays!

Overview:

Provides case management for patients and families to enhance their psychosocial functioning. Performs duties and responsibilities in a manner consistent with mission, values and Mercy Service Standards.

Qualifications:

• Education: Master's degree required in Social Work, Counseling or other human service related field.Licensure:
• Licensure: Licensed Master's Social Worker (LMSW), Provisional Licensed Professional Counselor (PLPC) or eligibility for licensure preferred.
• Experience: Minimum one year Behavioral Health experience preferred.
