We’re looking for an MDS Nurse at Ambassador Healthcare at College Park. The MDS Nurse will be responsible for coordinating the completion of the MDS process and supporting the care planning team.

MDS Nurse Duties & Responsibilities:

• Attend departmental meetings as part of the Nursing management team and interact effectively with all levels of Nursing staff
• Reach out to specialists within the community to get help in achieving care plan goals
• Assess and evaluate the quality of care being given to long-term care residents
• Follow the MDS/Care Planning schedule for non-Medicare residents
• Develop a care plan that has long-term and short-term goals for improvement
• Conduct meetings with multidisciplinary treatment teams, caretakers, and families
• Perform initial interview and assess new residents
• Conduct in-service education on MDS, RAPS, and Triggers

MDS Nurse Qualifications & Requirements:

• COVID Vaccination
• 2 years of experience in a Skilled Nursing Facility
• Valid RN or LPN in the State of Florida
• Knowledge of MDS, Medicare and care-planning process
