Job Description

About the Role

As our MDS Coordinator you will conduct and coordinate the development and completion of the Resident Assessment Document in accordance with the requirements of this state and the policies of this facility.

What You'll Do
As a member of our team you will have an impact on our clients through accomplishment of the following responsibilities:

  • Review unit charts daily to ensure the MDS documentation is being entered accurately by the Charge Nurse as required.
  • Ensures compliance with state and federal regulations in all areas of nursing care.
  • Maintains confidentiality in accordance with state/federal law as well as corporate policy.
  • Coordinate with the Administrator, Director of Nursing, and QAAC chair-person regarding issues noted on the Facility Quality Indicator Profile.
  • Makes daily rounds in the facility for the purposes of visibility, assessment, completion, and evaluation of care.
  • Provide the A.D.O.N.s a weekly charting "window" for their forthcoming MDSs as noted by the weekly MDS schedule generated and distributed by HIM.
  • Attend Quality Assessment and Assurance Committee meetings and other meetings as requested. Attend weekly Department Head and Utilization Review Meetings.
  • Input data onto the RAI instrument either via computer or long-hand in a manner that meets federal and state requirements.
  • Must hold an active LVN/RN and have an active license
  • Notify the A.D.O.N.s immediately of the issues noted upon review of the chart while doing the MDS. Make daily contact with the A.D.O.N.s to discuss any issues which might impact the MDS process.
  • Check with Health Information Management daily for critical updates.
  • Perform special assignments, projects, and other duties as required.
  • Coordinates liaison activities between the Nursing Department and other disciplines.
  • Notify the Director of Nursing of any issues germane to the functioning of the nursing units which may negatively impact patient care.
  • Check for distribution a minimum of twice daily.
  • Obtain weekly therapy updates from SNF Coordinator and Therapy Department for required MDS documentation.
  • Plans, organizes, and directs nursing personnel in fulfilling their responsibilities for nursing service.
  • Conduct in-service training for licensed staff as needed regarding the MDS procedure.
  • Analyzes and evaluates nursing and related services to improve the quality of elder care and maximize nursing time and abilities.
  • Seeks new ideas and fosters an environment of trust with staff, elders, and families.
  • Detects, corrects, or reports situations that have a possibility of causing accidents or injuries. Mentors, guides and assist C.N.A. by creating a team environment among all house members.
  • Ensures that the individual service plan is followed; administers or supervises treatments prescribed by physician.
  • Will assist in helping C.N.A. with elder care and household chores as needed or as time permits.
  • Perform general office tasks as required.
  • Work successfully as part of a team and responsible for developing then maintaining strong working relationships with co-workers.
  • Attend and participate in staff meetings and training as required.
  • Provide professional growth and development; develop and communicate goals and objectives;
  • Administers all intramuscular, sub-cutaneous injections; performs all in-house testing procedures; takes and records vital signs.
  • Works closely and cooperatively with therapist to promote the highest quality level of care for our Elders.
  • Address the RAI coordinator in Austin via e-mail with questions relating to the RAI process when the specific answer is not found in the RAI manual.
  • Maintain compliance with all Ventana by Buckner policies, procedures and requirements. Maintain compliance with all state and federal laws and regulatory requirements.
  • Adheres to the philosophy and concepts of the Small House Concept
  • Develops, trains and leads staff in an environment of customer focus.
  • Source, hire, supervise, monitor, evaluate and provide direction to direct reports, as needed.
  • Maintains awareness of internal operating management policies.
  • Maintains a safe work environment for all elders and co-workers
  • Collaborates with the Medical Director, Nurses, Consultants, therapists, in the execution of the elder care policies.
  • Monitor and provide constructive feedback; conduct annual performance evaluation.

What You'll Bring to the Team

  • Requires proficient ability to exhibit detail-oriented skills necessary to understand and manage a wide-range of information.

  • Proficient working knowledge and ability to accurately and timely operate and perform computer related tasks with specific equipment and software applications required.
  • Work deals mostly with areas such as preparing and reading data and figures, client records, medical orders, visual inspection involving small details, using measurements devices, dispensing medicines. Visual work is primarily close to observer. Depth perception and field of vision (peripheral) are not as critical as the ability to distinguish small details and markings very near to the observer. Color perception for some tasks (e.g., dispensing meds, assessment of elder condition) is important.
  • Must be a graduate of an accredited school of nursing. Must have a current licensure by the Texas State Board of Nursing. Experience in supervisory or leadership capacity in a hospital or long-term care facility, and/or geriatric nursing preferred. Current status as an RN required.
  • Requires ability to communicate verbally with elders, families, visitors and co-workers. Ability to make self clearly understood by others.
  • Requires ability to perform moderately difficult manipulation such as writing, typing; foot and hand-eye coordination required.
  • Requires ability to maintain confidentiality.
  • Requires ability to effectively manage personnel; requires administrative skills to include but not limited to staff selection, development, motivation, scheduling, and evaluation.
  • Requires ability to read, write and understand business documents which are written in English. Requires ability to speak English proficiently.
  • Requires ability to use up to 100 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people.

  • Requires ability to maintain composure, positive attitude and awareness of priorities in an environment of stress and frequent interruptions.
  • Requires ability to recognize differences in sound, such as voices/noises that are loud and playful instead of angry and combative.
  • Position requires bending, stooping, twisting, turning, reaching, lifting, carrying, pulling, pushing, climbing, kneeling, walking, and standing over 75% of shift.
