Our facility is mostly private pay, with a manageable 15-bed Medicare unit, and zero Medicaid beds!


Overview
Experienced MDS person will enjoy this small Medicare bed health center on a beautiful college like campus. The most important characteristic of our team members is that we love helping the aging population. Duties & Responsibilities

  • Teamwork and participation: maintains an effective relationship with all departments and professional staff; and participates on facility committees as assigned.
  • Informs residents/responsible parties of Medicare covered days and notifies them of their last covered day.
  • Updates MDS care plan schedules.
  • Transmits completed MDS forms to appropriate state and Federal agencies at required time frame, as required by regulation.
  • Helps maintain communication about Med A patients between therapy staff and Hoosier Village staff; (e.g., new admissions; admit and discharge dates; RUGS; care plan schedules; and occupational therapy, physical therapy, and speech therapy patient treatment times).
  • Maintains certification to give Mantoux tests, CPR, and first aid.
  • Keeps Medicare certifications up to date.
  • Completes computerized or written MDS forms and quarterly summaries within required time frame. Assures completion of the MDS by each department within the required time frame. Update care plans as needed.
  • Writes and/or assists the director of nursing in writing resident care plans for each resident. Chairs the care plan meetings.

Qualifications

  • At least two years of experience in geriatrics
  • Active license to practice in the State of Indiana as a LPN; RN preferred

As a condition of employment, Hoosier Village requires that all new hires provide evidence of full or vaccination against COVID-19 within 3 days of employment.
