Lvn Care Navigator (Sr. Lvn)

• Must provide documentation of graduation from an accredited school of vocational nursing, or completion of education and experience as a corpsman in the United States military.
• Current American Heart Association (AHA) CPR Certification.
• Two (2+) years or greater of LVN experience in the ambulatory setting.
• California Licensed Vocational Nurse (LVN).
• Must pass UCDH Pharmacology and Venipuncture verification exams.
• If required by position, must provide proof of valid IV Therapy Certification and must pass the UCDH IV Therapy verification exam.
• Applicants are encouraged to upload their license and certification if required for the position.

• Experience with health IT systems and reports.
• Local knowledge about and connections to community health care and social welfare resources.
• Knowledge of and/or proficiency in health behavior change or motivational interviewing.
• Knowledge of specific disease and lifestyle-related topics such as smoking cessation, weight management, nutrition, pre-post-natal care, stress reduction and chronic conditions.

• This position may be subject to a criminal background investigation, drug screen, Live Scan fingerprinting, medical evaluation clearance, and functional capacity assessment.
• Travel may be required to any UC Davis Health Clinic.
• Must be willing to work variable days (including weekends), hours, and locations as required to meet the business needs of the practice.
