LPN IN PRIMARY CARE CLINIC - E DDY SENIOR CARE - LATHAM

Weekdays 8-4:30pm NO WEEKENDS

Eddy SeniorCare is an exciting, innovative model of care for seniors, located at 385 Watervliet-Shaker Road, Latham. Our Program of All-Inclusive Care for the Elderly is located within Provincial House. We provide a team-based model of care including a physician, RNs, LPNs, Home Health Aides, physical/occupation/speech therapists, social workers, transportation, a day center team, and more!

Primary responsibilities include: working under the direction of a RN Supervisor serving as the Primary Care Clinic LPN and also providing in-home patient visits (travel is within a very limited area near our Center).

Requirements:

NYS LPN Licensure.

Experience in home health or a long term care facility preferred but not required. Must possess a passion for caring for seniors and demonstrate a respectful, caring, professional demeanor.
