We are seeking an Population Health - Life Manager, RN


Location: New Hope, MN


Schedule: Full Time: Monday – Friday; 8am-5pm CST with weekend rotation


Salary: $87k+, $0.62 mileage reimbursement, and $1,500 Sign on Bonus!

Responsibilities:


Using a value-based model in partnership with North Memorial Health and UCare, Lifespark utilizes the 7 essential elements of wellbeing to improve the lives of seniors and spark lives. This innovative program assists seniors to prevent re-hospitalization, address care gaps, and meet non-medical needs which are necessary to help seniors thrive.


The Life Manager is responsible to engage eligible members in the Lifespark Experience™ through proactive outreach and the implementation of Lifespark’s proven process. He/she works one-on-one with members to discover what is most important to them and develop a customized Life Plan that addresses all 7 elements of well-being. She/he manages the Life Plan, providing proactive guidance and connecting to essential 7 element services to deliver results. Acts within the full capacity of her/his licensure.


Qualifications:


  • Required active CPR Certification or ability to obtain certification upon start
  • 2 years’ RN experience in community-based services (assisted living, home care, hospice, public health)
  • In accordance with CMS Vaccine Mandate, candidate must provide proof of Covid-19 Vaccine or submit documentation for exemption
  • Prefer advanced certification as a Care Manager, Holistic Nursing, Geriatric CM, and/or Gerontology
  • Registered Nurse, BSN preferred
