The candidate will be working on a 16-week assignment at our skilled nursing facility in Bellevue.

  • $55 per hour
  • 2p - 10:30p / guaranteed hours (40 w/o benefits)

Summary of Responsibilities

  • Read Daily Communication Logs for resident information.
  • Ensure that proper Universal Precautions are practiced for resident and staff sanitation and safety.
  • Assist with personal care and physical support. This may include assistance after a fall, assistance in transferring, assistance with bathing, dressing, oral hygiene, toileting, ambulation, grooming, etc.
  • Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
  • Initiates preventive and rehabilitative nursing procedures as appropriate.
  • Along with the team, implement the nursing care plan for each patient
  • Complete resident treatments as ordered and scheduled.
  • Observes signs and symptoms and report to the physician and RN, reactions to treatments, including drugs and changes in the patient’s physical or emotional condition.
  • As appropriate, prepare residents’ medications to accompany them on short-term absences from the community as the state allows.
  • Understands and adheres to established policies and procedures.
  • Attends staff meetings, patient care conferences, and in-services as scheduled.
  • Document In residents’ Progress Notes any change in status. Review Progress Notes regularly.
  • Participates in medical record audit as assigned.

Requirements:

MINIMUM TRAINING AND EXPERIENCE

  • Acceptance of philosophy and goals of the Facility
  • Active BLS certification.
  • Graduate of a state-approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing.
  • Minimum of one (1) year experience in nursing, preferred.
  • Ability to exercise initiative and independent judgment.

OTHER SKILLS OR QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below represent the knowledge, skill, and/or ability required.

  • Refined deductive reasoning skills to be able to problem solve on shift
  • Ability to manage information electronically and transfer records appropriately
  • Appropriate competencies must be completed by the facility for any assignment.
  • Proficient in Microsoft Windows
  • Must have valid driver’s license or dependable access to a vehicle for business use
  • Must be in good health and physically/mentally capable of performing assigned tasks
  • Proficient computer skills in managing the electronic health record (EHR)
  • Ability to work in an electronic environment
  • Working knowledge of personal computing and word processing software

LPN’s are expected to perform duties of an LPN within their scope of practice. They shall be limited to performing only those functions for which they are competent and within their scope of practice. Assignments shall only include those duties and responsibilities where the LPN has demonstrated competency, and this has been verified by the receiving Careage facility
