* Salaries listed above include a 30% wage enhancement above base pay. Wage enhancements are for a specific period and are not part of the base pay.


Direct Care Additional $25 per week

Summary:

LICENSED PRACTICAL NURSE I
P erforms nursing care to residents within an assigned work unit under the supervision/direction of a Registered Nurse. Acts in accordance with the objectives set forth by the nursing department guidelines.
LICENSED PRACTICAL NURSE II
Performs certified level nursing care to residents within an assigned work unit under the supervision/direction of a Registered Nurse. May work in a charge nurse capacity and acts in accordance with the objectives set forth by the nursing department guidelines.

18 Months of additional relevant experience = Associate's degree

36 Months of additional relevant experience = Bachelor's degree

54 Months of additional relevant experience = Master's degree

Other information:

MINIMUM QUALIFICATIONS:

LICENSED PRACTICAL NURSE I

Education: Completion of a recognized school of practical nursing approved by the State Board of Nursing Education and Nurse Registration.

Experience: No experience required other than that obtained through required training.

License/Certification: Must possess a current license as a practical nurse or temporary permit issued by the New Hampshire Board of Nursing Education and Nurse Registration pending the results of the licensing examination.


LICENSED PRACTICAL NURSE II

Education: Completion of a recognized school of practical nursing approved by the State Board of Nursing Education and Nurse Registration.

Experience: Three years of experience as a Licensed Practical Nurse.

License/Certification: Must possess a current license as a practical nurse and be certified by the National Federation of Licensed Practical Nurses (NFLPN), Inc. or an organization recognized by the NFLPN, Inc. licensing examination.

SPECIAL REQUIREMENTS:

Employees may be promoted to the Licensed Practical Nurse II level based on the following requirements:

  • Successful performance evaluation.
  • Recommendation of the supervisor.
  • Completion of the experience and certification requirements of the LPN II level.

Responsibilities:

LICENSED PRACTICAL NURSE I

Documents provided nursing care in the residents’ records indicating problems, progress, and actions taken; describes application of prescribed treatments performed.

Administers prescribed medications and makes notations, before and after, of effects of medications given to guard against polypharmacy issues and mismanaged medication regimes.

Utilizes selective components of the nursing process to provide data for writing individual Plan-of-Care. Updates and revises care plans to reflect changes in resident status to ensure quality care.

Supervises LNA’s and makes periodic rounds of the unit to ascertain quality of care given and checks for cleanliness and resident safety.

Reports routine as well as unusual facts and situations to nurse manager or designee and receives report from off-going shift, transfers pertinent information to oncoming shift so that nursing care is provided in a consistent and continuing manner.

Discusses problems and changes in residents to Physician and allied consulting health professionals, provides assistance during their in-house visits to assure quality medical care.

Must provide nursing assistant support including but not limited to: direct ADL care, bathing, transfers, and lifting.

Educates residents to their care plan and courses of action to be taken to reach goals agreed upon at the resident’s Plan-of-Care conference to comply with residents’ rights to know regulations.

Briefs family members/guardians about the resident’s care, care plan and current progress and problems, alerts family/guardians of sudden changes noted with the resident to ensure continuity of information and family integration.

Admits, transfers and discharges residents as directed by attending physician to provide for timely medical care or least restrictive housing options.

Ensures compliance with established department policies and procedures by maintaining competency in Cardio Pulmonary Resuscitation (CPR) techniques.


LICENSED PRACTICAL NURSE II
