Position Overview

The Division of Developmental Disabilities is seeking to hire a Licensed Practical Nurse II for the Ludeman Developmental Center located in Park Forest, Illinois to perform skilled practical nursing care services for individuals, on an assigned shift, on an assigned unit. Administers prescribed medications, takes temperatures, blood pressures, pulses, and respirations. Documents administered medications and observes/reports any side effects on the Medication Administration Record (MAR) in accordance with the facilities policy and procedures. Develops monthly LPN (Licensed Practical Nurse) notes for assigned individuals. Completes medical reports in accordance with guidelines and policy. Participates in facility nursing meetings and unit meetings. On a rotational basis, assists in providing nursing care services to other units within the facility.

Job Responsibilities

1. Performs skilled practical nursing care services for individuals of the Ludeman Developmental Center on an assigned shift, on an assigned unit.

2. Implements physician orders as written.

3. Develops monthly LPN (Licensed Practical Nurse) notes for assigned individuals.

4. Completes Daily Nurses log and provides oncoming assigned nursing staff an updated report at the beginning of his/her shift.

5. Participates in facility nursing meetings and unit meetings.

6. Maintains the security, cleanliness, and organization of medication cabinets/carts for assigned unit in accordance with standards and guidelines.

7. On a rotation basis, provides professional nursing care services to other units within the Ludeman Developmental Center.

8. Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.

Minimum Qualifications

1. Requires Illinois license as a practical nurse.

2. Requires one year of practical nursing experience.

Conditions of Employment

1. Requires the ability to serve on call, after business hours, weekends and holidays on a rotation basis.

2. Requires the ability to utilize office equipment, including personal computers.

3. Requires the ability to physically restrain individuals as necessary to prevent injury to individual or others.

4. Requires the ability to travel in the performance of job duties, with overnight stays as appropriate.

5. Requires ability to pass the IDHS background check.

6. Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.

7. Requires the ability to meet all agency vaccine/health-related policies and guidance.
