Summary

The hiring salary range for this position is $4,191.66 - $5,029.76/month or $ $2,095.83 - $2,514.88/semi-monthly depending on qualifications.

Description of Work

This class specification represents only the core areas of responsibilities; specific position assignments will vary depending on the needs of the Division.

  • Evaluates and documents program eligibility status; determines case safety level, emergency status and/or need for crisis intervention or mandatory reporting; determines initial service needs at intake; monitors and documents client interaction while client is on program waiting lists and assist as needed when assigned for ongoing services.
  • Performs other duties of a similar nature and level as assigned.
  • Conduct analysis of client needs and implement person-centered individualized referrals.
  • Prepares, documents and maintains case and/or program information; collects, manages and analyzes data to produce reports documenting program measures and outcomes; provides recommendations; ensures the maintenance of timely program or case activity tracking, reporting and applicable billing.
  • Performs client interviews, intake, and assessments to determine service needs for clients who enroll into the ADRC programs; serves as an initial representative with other service providers to address service requirements, availability and needs assessment; makes resource referrals to both internal and external programs; provides client advocacy based on required situations.
  • Provides training, community education, and conducts program outreach for groups and individuals; researches and identifies community service needs; collaborates with stakeholders and makes program, process and community service recommendations; prepares and disseminates information. May be responsible for securing, monitoring and reporting special program funding.
  • Monitors client status while clients are on the program waiting lists; provides intervention support; conducts limited on-site or telephonic client visits and perform needs assessment; performs safety and risk assessments; determines crisis intervention needs; monitors services and makes plan adjustments as client needs change; escalates cases to program managers as required.
  • Coordinates the exchange of service information with other agencies; provides recommendations regarding course of action; assists in conflict or behavior management; and assists in the resolution of disagreements among clients, families, systems and service providers.
  • Receive, coordinate, and manage referrals coming into the Extended Information and Assistance, Options Counseling, Case Management, and Elder Refugee Programs (herein referred to as ADRC Programs). Document in various tracking systems, communicate, and follow up on referrals as appropriate. Manage the referral processes for internal specified ADRC programs.

  • Coordinating incoming referrals and coordinating service referrals
  • Record management principles;
  • Conducting individual initial needs assessments,
  • Interpreting, monitoring and reporting information and statistics.
  • Providing consultation and case coordination for older adults while during the intake and enrollment process and when they are on the program waiting lists both in person (during a home visit) and over the phone. This may include the following:
    • conducting individual initial needs assessments,
    • working with older adults and their families to identify options to address problems,
    • contacting selected agencies to set up service,
    • following up with clients to ensure needs are met.
  • Principles and methods of qualitative and quantitative research;
  • Maintaining program or case information.
  • Conducting interviews to determine client or program service needs and/or intervention.
  • Researching industry trends, solutions and best practices.
  • Modern budget practices;
  • Utilizing a computer and relevant software applications.
  • Providing face to face or telephonic information consultation (note: face to face interactions is limited) and referral to clients regarding the resources and services available to older persons and their families;
  • Exercising confidentiality.
  • Contacting selected agencies to set up service,
  • Aging network and community resources available in the region;
  • Developing relationships with referral sources and provide education and outreach regarding AAA services;
  • Applicable federal, state and local laws, codes, regulations (based on assignment);
  • Working in tandem with staff to facilitate the full range of community-based service delivery options to older adults and adults with disabilities to increase access to benefits and resource needs.
  • Being responsible for coordinating referral processes into the applicable ADRC programs and updating staff, forms, and documentation as needed;
  • Principles and practices of program administration and management;
  • Best practices, trends and emerging technologies;
  • Providing customer service.
  • Gathering data, analyzing findings and applying logic and reason.
  • Compiling and sorting data and articulating issues and recommendations.
  • Utilizing communication and interpersonal skills as applied to interaction with coworkers, supervisors, the general public and others to sufficiently exchange or convey information and to receive work direction.
  • Following up with clients to ensure needs are met.
  • Conducting face to face or virtual visits with community resource organizations to increase knowledge of services and allow for “warm transfers” of consumers during referral;
  • Bilingual English/Spanish preferred.
  • Specialized equipment relevant to area of assignment;
  • Enhancing and maintaining agency resource information of organizations that serve older adults;

Education and Experience

  • Bachelor’s degree in gerontology, social work or related human services field and three to five years of related human services experience. OR An equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as those listed above, unless otherwise subject to any other requirements set forth in law or regulation.

Necessary Special Requirements:

Depending on assignment, some positions may require:

  • Valid Colorado Driver’s License and access to an automobile or to substantively equivalent alternative transportation.
