Job Description
Location: Fort Collins, CO 80525
Status: Full Time
Hourly rates: $17-$19 per hour
Shifts: Monday-Friday; 8:00am-5:00pm, with on call responsibilities
Interim HealthCare is looking for an Intake Coordinator. We are a health care company that has been in business locally for 29 years and is still growing strong! This is a key role in our organization, and we are looking for someone who can make a great first impression and can follow up with excellent customer service. This is a great opportunity to join a home health team of a long-standing, stable and reputable company.
Responsibilities Include (but not limited to):
  • Completes other assignments as requested and assigned.
  • Acts as a contact telephone liaison person interfacing with referral sources, outside agencies, health professionals and field staff.
  • Completes intake screening including obtaining, documenting, and analyzing all required information.
  • Communicates with patient/client and family regarding arrangements for the initiation of care/services.
  • Receives and logs referrals. Prepares reports and responses to inquiries. Provides telephonic contact and system data entry for customers seeking care/services.
  • Collects and enters customer information into the management information system.
  • Participates in on-call rotation, including 1 weekend per month.
  • Receives and coordinates all incoming calls, providing patients/clients and referral sources with basic information to assist in accessing appropriate services
Job Requirements
  • Experience interfacing with referral sources, outside agencies, health professionals, and field staff.
  • Covid Vaccination Card OR medical OR religious exemption
  • Strong organizations skills: ability to manage multiple tasks, prioritize and work independently in a fast paced, high-volume environment and focus on details.
  • Knowledge of medical terminology.
  • Excellent customer service, phone, computer, and data entry skills.
  • Computer proficiency to include current company software programs.
  • Ability to develop strong working relationships and works well with a team.
  • Previous home care/ health care / medical office experience preferred including experience and knowledge of insurance authorizations, admissions, or intake processes.
  • Pass a background check and a drug test (we are required to test for THC)
