Position Summary
:

The infectious Disease Clinical Pharmacist provides collaborative pharmaceutical care to hospital patients through the Antibiotic Stewardship Program with an Infectious Disease physician, faculty and house staff physicians, pharmacists, and clinical microbiology staff.


Major Position Responsibilities
:

The position characteristics reflect the most important duties, responsibilities and competencies considered necessary to perform the essential functions of the job in a fully competent manner.   • Participate in selection of patients receiving targeted antimicrobials and monitor these patients on a daily basis.
  • Document clinical consultations and cost avoidance as part of the justification for the position/service.
  • Maintains a safe, clean working environment, including unit based safety and infection control requirements.
  • Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, duration, therapeutic duplication, drug interactions. Switch patients IV to PO therapy when appropriate.
  • Provide pharmacokinetic consultations and clinical consultations as required.
  • Promote the use of the formulary when possible, but coordinates procurement of non-formulary drugs when necessary.
  • Provide cost-effective pharmaceutical care to patients receiving targeted antimicrobial therapy.
  • Discuss medication order changes/clarifications with the infectious disease physician or the prescriber, document any changes in patient and pharmacy records, and inform others of medication order changes.
  • Communicate with pharmacy to avoid waste in preparation of antimicrobials.
  • Detect, monitor, document and report adverse drug reactions and medications errors.
  • Develop and conduct target drug programs and drug usage evaluations as needed and report results to the Pharmacy and Therapeutics Committee. Represent pharmacy on infection control committee.
  • Participate in appropriate departmental quality improvement and education initiatives.
  • Provide operational support to the pharmacy when needed.
  • Educate medical and pharmacy staff about antimicrobials.


Qualifications:

  • PGY1 or 3 to 5 years equivalent inpatient experience.
  • Knowledge of infectious diseases, treatment and surveillance strategies.
  • Ability to assist with cost saving initiatives and patient care optimization with respect to medications.

Education:

  • Post Graduate Year 1 (PGY1) Residency, required.
  • PharmD, preferred.
  • Post Graduate Year 2 (PGY2) Residency, preferred.

Licenses:

  • Current license by the California State Board of Pharmacy, required.

Certification:

  • ACLS or PhART (Pharmacy Advanced Resuscitation Training), required.
  • Board Certified Pharmacotherapy Specialist (BCPS), preferred.


Each new hire candidate who is offered employment must pass a physical evaluation, urine drug screen and pre-employment background checks before starting work.

  • Salary/Hourly Wage range based on experience.
