Job Description
Overview


VNA Health Group is a premiere Medicare/Medicaid-certified provider of home health , hospice , and community-based services. We’ve been serving families and individuals with the most advanced and comprehensive clinical home care in NJ since 1912.


We are currently looking for a Hospice Liaison Coordinator to join our team. The position would be responsible for assessing patients for hospice needs and transition of service and coordinating total care for hospice patients and their families.


Responsibilities


  • Adheres to agency infection control and safety policies
  • Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position
  • Adheres to the organization’s policy in regards to absenteeism and appearance
  • Assists in managing and monitoring the quality and appropriateness of the services and programs
  • Conducts community outreach activities to improve community access to hospice care
  • Assists in developing and implementing a discharge program
  • Assesses the needs of the patients and provides counseling to patients and their families and coordinates the transition to hospice to promote continuity of care
  • Establishes and maintains liaison with physicians, external and internal customers
  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers


Qualifications


  • Excellent clinical skills and experience in hospice nursing and community health nursing
  • Bachelor’s degree in Nursing Preffered
  • Excellent customer service, organizational and interpersonal skills
  • Strong computer skills with proficiency in MS Office Suite
  • New Jersey Registered Nurse or LPN license Preferred or similar role within a healthcare organization.
  • Excellent verbal and written communication skills


Working Conditions/Physical Demand : Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and driver’s license
