Job Description
Under limited supervision, performs outreach and builds relationships with homeless neighbors; provides advocacy; and assist with connection to social services and permanent housing; and performs other related duties and special projects as assigned. Must be available to work evenings and or weekends as needed.

DUTIES AND RESPONSIBILITIES (which are not in any hierarchical order)

1. Identifies and assists unsheltered homeless neighbors through direct street outreach activities and/or through community referrals.

2. Works collaboratively with the Homeless Services Coordinator and other agencies to conduct outreach in the community, focusing on those who are most vulnerable. Outreach will occur in identified "hot spot" neighborhoods within City at times when individuals are most likely to be found (including early morning, evenings, and weekends as needed).

3. Provides linkage to other needed resources, such as mental health, substance abuse, legal, assistance with housing and benefits applications, distribution of health and hygiene supplies, and transportation assistance.

4. Establish and maintain positive, productive working relationships with clients as well as mental health organizations, shelter programs, police (and other local officials), and providers of services and resources to homeless neighbors.

5. Completes Vulnerability Index Service Prioritization and Decision Assistance Tool (VI-SPDAT) to determine vulnerability and acuity of neighbors during street outreach as well as other outreach and referral processes.

6. Documents all contacts and outcomes in the Homeless Management Information System (HMIS) and any other system as assigned; maintain complete client records, daily activity logs, mileage logs, and other reports as directed.

7. Provides advocacy for homeless neighbors when they encounter barriers.

8. Assists in obtaining housing readiness documentation including but not limited to ID, social security cards, and income verifications; assists in identifying appropriate housing and completing housing applications.

9. Coordinates transportation for clients to and from appointments.

10. Attends team meetings, case conferences, training workshops, taskforces, and other community meetings as required.

11. Coordinates and participate in community outreach events including but not limited to the Point-In-Time counts.

12. Performs other duties or special projects as assigned.



Salary


$20.11 Hourly (Pay grade 46)

Qualifications

Associate degree with a major in Social Work, Psychology, Behavioral Science, or related field, and two (2) years of homeless outreach, social work, or any equivalent combination of training and experience.

Bilingual Spanish or Creole is highly desirable.

Alternate Requirement:
Bachelor's degree with a major in Social Work, Psychology, Behavioral Science or related field required.

A valid State of Florida driver's license is required. A driver's license from another state (equivalent to State of Florida Class E) may be utilized upon application with ability to obtain the State of Florida driver's license within thirty (30) days from date of employment.

Education Requirements
: To be considered valid (or verified) for position qualification purposes, and consistent with City Hiring Policy 4-5, a High School, GED or equivalent diploma, and all higher education degrees or certifications must have been issued by:
  • An institution accredited by an accrediting organization that is recognized by the Florida Department of Education or the U.S. Department of Education; or
  • A public education institution in the State of Florida or any State or Territory of the United States; or
  • An institution whose degree or certification is accepted by institutions identified in either a), b), or c), above, for the purpose of admission to institutions identified in either a), b), or c) above.
  • An institution that is accredited by an organization that is recognized by the Council for Higher Education Accreditation (CHEA).
International degrees will be evaluated to determine if they satisfy qualification requirements.

The selection process for this position may consist of any, all or a combination of the following tests: evaluation of training and experience, written examination, structured oral interview or other assessment method.

State of Emergency:
When normal operations of City departments are suspended or interrupted due to a declaration by the Mayor that a state of emergency exists, all employees will be deemed critical by the City. For the preparation and/or continuation of emergency operations or for special work detail, employees deemed necessary shall be required to work. Employees deemed critical by the City but not needed for the initial preparation and/or continuation of emergency operations or for special work detail may be directed not to report to work, and given paid administrative leave or they may be reassigned to another location that best meet the needs of the City.

The City of West Palm Beach allows remote work assignments for certain job classifications and under certain conditions in accordance with Remote Work Policy 4-20.
