Organization Description

Since 2005, Pat’s Place Child Advocacy Center has served as a multidisciplinary hub, bringing together detectives, child protection workers, medical and mental health providers, prosecutors to respond collaboratively to child sexual abuse allegations.

Our mission is to protect and heal children, unite key partners, and engage the community to end child abuse.

The High-Risk Youth program at Pat’s Place is an initiative to provide services to youth who have experienced child abuse and have risk factors that place them at high risk of human trafficking and exploitation.

Position Title: High-Risk Youth Case Manager/Bilingual (Full-time/Non-exempt)

Reports To: Director of Education and Community Outreach

Starting Salary Range: $42,000-47,000 annually

Position Description

The High-Risk Youth (HRY) Case Manager will work in partnership with youth ages 12-17 who have experienced abuse and are at high risk of human trafficking and exploitation. The Case Manager will provide relationship-focused, client-centered, low barrier services that respond to clients' needs for safety, housing, basic needs, connections with family and community, and mental and physical health care. The HRY Case Manager will coordinate with all participating agencies and service providers to ensure a strong, cohesive and collaborative approach to high-risk cases.

Responsibilities

  • Provide case management in the office, home, and/or community.
  • Input survey data and track outcomes.
  • Provide wraparound service planning and systems navigation to assist youth and their families when interacting with complex social services, health service, criminal justice, and/or immigration systems.
  • Screen cases referred to PPCAC to identify high risk clients appropriate for HRY services.
  • Provide support group leadership.
  • Other duties as assigned.
  • Conduct needs assessment using various assessment tools to appraise and interpret a runaway and high-risk client’s risk factors and needs.
  • Collaborate and coordinate services with external partners to provide a continuum of care for clients.


Work Hours and Practice Location:

  • Expected to work on-site unless negotiated with direct supervisor.
  • Travel throughout the community is required for daily job duties.
  • Pat’s Place Child Advocacy Center recognizes a 37.5 hour work week.

Requirements:

Required Qualifications

  • Bachelor's degree in Social Work or related field, or commensurate experience
  • Bilingual (Spanish/English)

Preferred Qualifications

Two or more years’ experience in child welfare, anti-trafficking efforts, or in a child advocacy center
