Job Description
Position Summary


The Home Health Quality Coordinator is responsible for modeling the 3 Company values of Compassion, Integrity, and Excellence, and for promoting the Compassus philosophy, using the 6 Pillars of success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Home Health Quality Coordinator assesses the quality of care provided to home health patients, families, and staff. S/he audits documentation, compiles and reviews clinical performance data for quality and regulatory compliance, participates in activities to support survey readiness, provides 1:1 education to support clinical development and positive outcomes, and works closely with Program leadership. S/he is responsible for applying the rules and regulations of state and federal regulatory agencies and other certified agencies.


Position Specific Responsibilities


  • Assists with Plan of Correction audits following internal/external surveys/audits.
  • Performs other duties as assigned.
  • Assesses and/or audits clinical documentation to ensure documentation reflects quality of care and services provided.
  • Performs focused audits at regular intervals to assess for compliance and support QAPI initiatives, and to ensure constant survey readiness.
  • Keeps informed of current Medicare/Medicaid regulations.
  • Attends and participates in staff meetings as requested to provide education on quality, OASIS documentation, regulatory updates and QAPI initiatives.
  • Reviews clinical documentation of OASIS assessment and plans of care and identifies educational opportunity for better understanding of OASIS guidelines.
  • Reports deficiencies to appropriate staff and supervisor for corrections.
  • Works with Regional Quality Director – Home Health in deficient areas to monitor effectiveness of resolution.
  • Communicates identified needs and potential solutions to supervisor.
  • Reviews clinical documentation to ensure it meets Medicare/Medicaid regulations.
  • Assists in chart reviews, as requested by fiscal intermediary.
  • Provides education to colleagues/clinical teams to support improved OASIS documentation and optimal outcomes.


Education And/or Experience


  • At least one (1) year of experience with point of service electronic documentation, HCHB preferred.
  • Bachelor’s degree in Nursing or Therapy required.
  • Minimum of one (1) year experience in home health QAPI or QA related field preferred.
  • Minimum of three (3) years of recent home care experience required.


Skills


  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, colleagues, investors, and external parties. Strong written and verbal communications.


Certifications, Licenses and Registrations


  • Active and unencumbered Registered Nurse or Therapist license in state(s) of employment required.
  • Current certification as a home care-coding specialist and OASIS certification preferred.

Estimated salary range $63835 - $86860 / year. Actual salary will vary by geographic location and experience.
