Healthcare Coordinator Jobs

Under the supervision and direction of the Director of Clinical Services, the Healthcare Coordinator serves as key liaison, advocate, and resource of information regarding routine and special healthcare for each person supported by the agency. This position provides clinical consultation regarding people with developmental disabilities supported by the agency, with specific attention to individual health risk factors whose mitigation impacts diet, medication and/or medical treatment/medical safeguarding devices. This position also provides assistance to the Continuous Quality Improvement process including management of the Medication Committee. Requires clinical competencies and experience in the health care field (Registered Nurse RN, and preferably experience with special needs individuals). Provides clinical services to ensure individuals in care are receiving relevant, competent, and compassionate care that meets their needs. Provides valuable input into the agency's continuous quality improvement efforts to ensure the it maintains full adherence to regulatory policies and procedures. Maintains timely, detailed documentation to ensure adequate funding levels are met. Records individual progress which will ensure continuity of support to individuals served.
