POSITION PURPOSE

The Health Information Management (HIM) Technician ensures the medical record is accessible and maintained for patient care and hospital operations. The HIM Technician is responsible for medical record retrieval, review, assembly, verification and coordination. The HIM Technician supports the HIM department through a variety of complex document imaging functions including prepping and scanning of records and quality control checks. Comprehensive HIM department functions include assisting with responding to requests for information from internal and external customers, maintaining confidentiality of protected health information and record storage.

ESSENTIAL FUNCTIONS

Knows, understands, incorporates, and demonstrates the mission, vision, and values of the Ministry in behaviors, practices, and decisions. Retrieves the medical record throughout the hospital as directed, verifying receipt of record against daily discharge reports. Ensures all records are collected within defined timeframes post discharge. Assembles and prepares records for the scanning process into defined order following established processes. Checks the record thoroughly to ensure the correct patient and encounter information are present. Scans the documents into the electronic medical record system. Document imaging tasks include processing inpatient records, outpatient records, clinic records, and loose documents in the electronic medical record systems. Ensure that the quality of scanned images, turnaround times and productivity are meeting established standards at all times. Perform quality control checks by reviewing each page are prepped and scanned correctly. Files and stores medical records post-scan according to retention policy. Purges and prepares medical records for destruction as directed by established policy. Serve as a resource for the department on releasing patient confidential information appropriately and within regulatory guidelines. Respond to medical record requests for continuity of care in a timely manner and applies HIM and appropriate policies and procedures governed by HIPAA, Joint Commission, Medicare and other state and local regulations and laws in the interpretation of PHI requests by internal and external customers and for all disclosures. Answer phones and assist with customer inquiries. Receives and greets HIM department visitors, provides general information, notifies appropriate personnel and directs deliveries to appropriate areas. Perform accurate and timely completion of birth certificates, affidavits of paternity and death certificates as directed. Electronically submit the completed documents to the State per regulation. Maintains knowledge of all policies and procedures, laws, regulations and accreditation standards regarding the legal health record. Conduct other duties as requested by supervisor or management.

MINIMUM QUALIFICATIONS

  • Minimum of one year experience in health information management, comparable physician office, clinic or hospital business office, customer service/office setting, or applicable college education required. Experience with electronic medical records preferred.
  • Certification as a Registered Health Information Technician (RHIT) or certification-eligible, preferred, normally achieved by successfully completing two-year college level coursework in an RHIT program.
  • High School Graduate or GED equivalent
  • Office organizational skills including accurate filing. Knowledge of medical terminology preferred.
  • Ability to use a standard desktop and windows-based computer system, including a basic understanding of email, Internet and computer navigation. Ability to use other software as required to perform the essential functions of the job.
  • Excellent organization skills and can perform several duties and functions related to the daily operations of health record completion and demonstrate excellent customer service skills.
  • Effective communication skills, both written and oral. Able to communicate effectively with individuals and groups representing diverse perspectives. Understands when escalation of issues is necessary. Ability to train and educate co-workers.
  • Skills and knowledge to provide general office coverage including but not limited to: answering phones and responding to requests for information.

PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS

Must possess the ability to comply with Trinity Health policies and procedures. Must be able to perform detailed tasks and handle frequent interruptions, as well as have the ability to change and be flexible with work priorities. Ability to collaborate with team members to organize work, and set priorities to complete assigned workload, team goal, and handle multiple demands. Must be able to spend majority of work time utilizing a computer, monitor and keyboard. Involves a wide array of physical activities, primarily walking, standing, balancing, sitting, squatting and reading. Must be able to move and control cart holding up to 20 pounds. Must be able to perform some lifting and/or pushing/pulling up to 20 pounds.
