Community Healthlink (CHL) is looking for full-time (37.5 hrs/wk) Family Support and Training Manager in Worcester.


Families and Communities Together (FCT) is a collaboration between Community Healthlink, LUK, Inc, and Communities of Care that oversees 3 wraparound programs operating in the Central MA area as part of the Commonwealth's Children's Behavioral Health Initiative. The FCT Wraparound Programs seek to build on family's strengths and available support systems to help children with serious emotional disturbance remain in or return to their communities. The FCT staff work with a family to develop a Care Planning Team (CPT) which consists of the child, family, people invited by the family, FCT staff and appropriate state agency and school staff. The CPT develops an Individual Care Plan that focuses on the child's and family's needs and proposes ways to improve the child's well-being at home, at school and in the community.


  • Assists with ongoing quality improvement, including the direct observation of the work of FCT Family Partners, as well as collection of data using Community Healthlink Quality Assurance tools.
  • Receives complaints or concerns from enrolled families, providers, state agency representatives, or anyone who expresses concern about Family Partners and/or the FS&T service process and works with the FCT Program Director to remedy such issues.
  • Respects diverse views and approaches demonstrates Standards of Respect and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.
  • Perform other related job duties as required and necessary
  • Follow all policies and procedures of the agency.
  • Oversees day to day operation of all Family Support & Training activities in FCT and ensures all Family Support and Training services are in compliance with all contractual, licensing and agency policies and procedures.
  • Bachelor’s degree in a human services related field from an accredited university or college and one (1) year of experience working with youth/families; or an Associate’s degree in a human services related field from an accredited school and one (1) year of experience working with youth/families; or High School diploma or GED and a minimum of two (2) years’ experience working with youth/families.
  • May carry a small FS&T caseload, as needed

  • In partnership with the FCT Program Director, convenes regular administrative staff meetings to address issues of morale, work environment, policies, and procedures.
  • Oversees utilization management and assist as needed in accreditation processes.
  • Provides direct administrative and clinical supervision for Family Partner Supervisor and Family Partners (with assistance and oversight from FCT Program Director).
  • Has a current valid US-issued driver’s license and ability to provide registered and insured automobile for work related purposes including transporting clients
  • Participates as appropriate in all management meetings, training, evaluation/research, supervision, quality management, and related activities as may be directed by the FCT Program Director.
  • Identifies ongoing training needs for the FCT staff, and work with the FCT leadership team to meet these needs.
  • Helps facilitate the local System of Care meetings.
  • Attends statewide CSA meetings, and other local and state meetings as needed and directed by the FCT Program Director.
  • Assist the FCT leadership team with development and maintenance of FCT policies and procedures.
  • Experience in navigating and working with any of the child/family serving systems or teaching other parents/caregivers to navigate such systems.
  • Assists in the recruitment and orientation of new FCT staff.
  • Provides essential leadership in the creation and maintenance of a positive and collaborative working environment where all FCT employees and enrolled youth and their families feel welcome, supported, and comfortable.
  • Experience as a caregiver of a youth with special needs—preferably a youth with mental health needs.
  • Must have a minimum of two (2) years’ experience supervising staff within a child/family serving human services program.
  • Experience providing Family Partner services within a Wraparound Service Planning Process.
  • Work assigned hours, which may include some weekend and evening hours.
