CereCore is seeking an Epic Healthy Planet Population Health Analyst to join one of our healthcare clients in Jupiter, FL (Hybrid).

Responsibilities:

• Working with Epic representatives, your organization's business community, and end users to ensure the system meets the organization's business needs regarding the project deliverables and timeline.
• Serving as a liaison between end users' workflow needs and Epic implementation staff.
• Holding weekly communications with team members to discuss the status of deliverables, shared issues, end‐user concerns, budget, and upcoming milestones.
• Developing an understanding of operational needs to set the direction for the organization's workflows by attending site visits and other integrated sessions.
• Maintaining regular communication with Epic representatives, including participating in weekly project team meetings.
• Guiding workflow design, building, and testing the system, and analyzing other technical issues associated with Epic software.
• Participating in training and working with end users. Troubleshooting problems and questions.
• Identifying and implementing requested changes to the system.
• Reviewing the status of projects and issues on an ongoing basis with leadership.
• Acting as the primary support contact for the application's end‐users. Identifying issues that arise in their application area as well as issues that impact other application teams and working to resolve them.

Position Requirements:

• Work history/knowledge of population health concepts and workflows.
• Prior analyst experience working with Population Health and case/care management systems.
• Familiarity or background in quality reporting initiatives such as PQRS, ACOs/MSSP, and MU. Familiarity with health plan concepts and workflows.
• Bachelor’s Degree preferred or a combination of college education studies and/or relevant experience.
• Hybrid role (expected to be in the office 3 days a week)
• Epic Certified in Healthy Planet & Compass Rose.
• Work history/knowledge of case/care management workflows and wellness programs.
• Prior experience with the Epic Healthy Planet Population Health solution is a plus.
• Proficient in MS Office applications.
