Community Nurse Jobs

This part-time position helps to coordinate and deliver health care-related services to individuals and families in the Hanover community, with a focus on home visits, teaching and coaching, and promoting both health and wellness and disease prevention. This position is not intended to substitute for either medical care or skilled nursing care. The Community Nurse uses his or her nursing skills, experience and judgment in evaluating individuals’ health care needs and referring them to appropriate health care professionals and community resources and services. All duties performed are within the scope of the New Hampshire Nurse Practice Act.

The focus of the community nurse is on home visiting, teaching, coaching, and integration of services as well as prevention of disease and promotion of health for individuals and the community.

• Educating the community about health, nutrition, hygiene, and disease prevention
• Assessing the needs of the community
• Making referrals to other organizations including area on aging, food banks, and social service agencies
• Developing care plans
• Attending Community Nurse Connection monthly support meetings via zoom
• Documenting interventions and outcomes in REDCap system
• Assisting with advanced care planning
• Making home visit to assess physical and social well being

Bachelors in Nursing preferred, active NH RN license

Strong health assessment skills
Maturity in the nursing profession
Excellent time management skills
Knowledge and skills associated with 5 Plus years of experience in nursing care.
Home care nursing desirable but not required.
