Job Description
Responsibilities:
Title: Community Mental Health Outreach Worker (West Region)
The Community Mental Health Outreach Worker (CHW) provides services to address social determinants of health (SDOH) needs identified during screenings. Provide members and caregivers education on mental health issues, wellness, self-care, community resources, access to services, among others. Assists member and caregiver to have access to the services and resources necessary to support the members recovery process. Works with and supports family members/caregivers through member’s treatment and recovery.
  • Verifies the member eligibility before providing services.
  • Conduct the interview process and documents according to requirements of the Health Insurance Portability and Accountability Act (HIPAA) and Law 408.
  • Conduct telephone or personal interview to gather the necessary data, document health care services, coordinate appointments and makes the referrals.
  • Identify which psychosocial problems affecting the population served are.
  • Upon answering the telephone, FHC staff identifies by name, title, and organization’s name.
  • Assume responsibility for the delegated task and provides follow-up.
  • Other duties
  • For initial screening, the organization limits use of non-clinical administrative staff to:
    • Performance of review of service request for completeness of
    • Acquisition of structured clinical data; (Structured clinical data is collected using explicit script or algorithms).
    • Collection and transfer of non-clinical data.
    • Activities that do not require evaluation or interpretation of clinical
  • Develop educational materials for Employee Assistance Programs and other FHC clients.
  • Provides the basis for planning the educational activities.
  • Participates in annual training of the Company, such as: Annual Compliance
  • Provide services to caregivers through the Apoyo al Cuidador initiative.
  • Performance of review of service request for completeness of
  • Shows appropriate communication skills to deal with a difficult client.
  • Use research to improve the health educational practice.
  • Divides time by setting priorities in tune with the needs of the organization and maintaining flexibility for the unexpected.
  • Answer the telephone with a clear and pleasant tone of voice, and with an appropriate language expression.
  • Identify the resources available in the community to coordinate referrals and orient members about the services addressing SDOH.
  • Participate in health fairs.
  • Develop and present emotional wellness educational workshops, conferences, or presentations.
  • Maintains a professional behavior, follows the code of conduct, in accordance with the policies established in the institution.
  • Maintains the confidentiality of the documents and the information received.

Qualifications:

Minimum qualifications:

Education: Bachelor’s degree in social work, Psychology, Counseling, or any other health related
Relevant Work Experience: One year or more of experience, preferable in health
Knowledge of administrative office procedures such as filing, designing forms, and other office
Preferable bilingual(Spanish and English).
Computers – Knowledge of software, such as Microsoft Office and Outlook
