Summary:
The Community Health Worker (CHW) serves as a consistent care coordinator throughout the continuum of care for patients and families throughout their healthcare journey. The CHW helps patients communicate with their healthcare providers to ensure they receive the information needed to make decisions about their health and social needs. The incumbent coordinates communication with providers and other healthcare team members on behalf of the patient. The CHW promotes, maintains and improves population health by fostering connections that bridge systems of health, social services, community based organizations and individuals by addressing social drivers of health and improving psychosocial well-being.
Education Requirement:
High School diploma, or equivalent required. Bachelor's Degree in a Human Service field preferred.
Experience Requirement:
One year of experience working in a community-based, multi-cultural setting required (combination of health/social services experience and/or education may be substituted.)
Licensure/Certification Requirement:
Basic Life Support required (BLS certification will be obtained as part of onboarding if not already certified). Additional training/certification may be required.
Key Job Responsibilities:
- Manages health care records
- Organizes schedules and manages appointments for patients to ensure they receive services in a timely manner.
- Identifies and communicates any social challenges faced by the patient.
- Facilitates interpretive language services when needed.
- Facilitates improved health care access and quality for underserved populations through advocacy and care coordination.
- Informs the patient on financial aspects of care and linking patient to financial resources as needed.
- Effectively manages time, workload and resources.
- Creates a seamless flow through the health system to facilitate the patient’s care.
- Maintains visibility and accessibility to assigned patient base.
- Works collaboratively with all members of the healthcare team to meet the patient needs.
- Uses approved screening tool to document SDOH and facilitates referrals to community agencies.
- Performs other job functions as assigned or requested.
- Facilitates communication between the patient, family members, and healthcare providers to ensure patient satisfaction and quality of care.
- Proactively manages relationships with patients.
- Manages follow up visits and ensuring continuation of care and linkage to community resources.

Supervision Provided by this Job:
- There are no supervisory or lead responsibilities associated with this role.
Physical Demands/Conditions:
- The physical requirements of this position are: medium work - exerting up to 50 lbs. of force occasionally and/or up to 25 lbs. of force frequently.
- Frequent invasive and non-invasive patient contact.
- Ability to react quickly to emergency situations.
- Must have good balance and coordination.
- The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
- The employee is required to have close visual acuity including color, depth perception, and field of vision to perform an activity, such as assessing patients, preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
