Tilman J. Fertitta Family College of Medicine
Helps move program efforts forward with the UH Community Health Worker (CHW) initiative by providing outreach, navigation and eligibility assistance for participants of various programs.
- Serves as point of contact between community/neighborhood and the UH CHW initiative.
- Establishes rapport with community members and families participating in the UH CHW initiatives.
- Recruits program participants for study.
- Conducts home/site visits to identify needs for services and provide information and referrals to diverse resources.
- Maintains case record documents providing thorough documentation in a timely manner.
- Serves as peer trainer for new or less experienced staff.
- Partners with CHWs and staff members to assist in contacting community members participating in UH initiatives.
- Adheres to policies and procedures of the organization and CHW Initiative programs.
- Maintains partnerships with community based organizations (local schools/ school districts or health and community non-profits or stakeholders in the CHWs community/neighborhood.
- Performs other job-related duties as required.

The position will work with the community from the Third Ward and East End as part of the Household Centered Care Program for the Tilman J. Fertitta Family College of Medicine in the Office of Community Health to connect community members with health coverage, medical and mental health services, food,and other social needs. The work will be conducted with an interprofessional team of medical, nursing, and social work students. This position requires reliable transportation as well as evening and weekend hours as needed, advance notice will be provided for changes in work schedule.
CHW‐I ‐ will automatically move to a CHW 1 upon receipt of certification. Certification required within one year of employment.

Community Health Worker Intern-
High School Diploma and No experience necessary or certification required.
Community Health Worker I-
High School Diploma- no experience necessary and Health Worker Certification must be obtained within at least the last 12 months.
Community Health Worker II-
High School Diploma and 1 year of experience, and Health Worker Certification obtained within at least the last 12 months.
Community Health Worker III-
High School Diploma and 3 years of experience, and Health Worker Certification obtained within at least the last 12 months.
Education: Requires a basic knowledge of grammar, spelling, punctuation and simple mathematical functions like percentages, ratios, etc. as might normally be acquired through attainment of a high school diploma or GED.
