Position Responsibilities: Provide a high level of customer service in assisting Home Care Staff with administrative and consumer related functions to support the work of Care Managers and Nurses, including home visits to consumers. Position includes home visits in the 28 cities and towns in the Merrimack Valley and North Shore.


Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)


  • Attend mandatory meetings and trainings including General Staff Meeting and Home Care Staff Meeting.
  • Other duties as assigned by Home Care Management Staff.
  • Assist with making telephone calls to consumers or family members to schedule appointments, and/or provide interpretation services for in-home assessments and complete limited paperwork/consumer documentation as needed.
  • Observe all confidentiality regulations and protect consumer information.
  • Complete administrative functions as needed.
  • Assist consumers in completing applications for public benefits programs including MassHealth, Fuel Assistance, SNAP, and Housing.
  • Assist consumers who are requesting emergency purchases such as prescriptions, food or other necessary items by procuring and delivering required items to the consumer.


Qualifications: Bilingual in Spanish required. Associates Degree and 1 year relevant work preferred. High School diploma with significant relevant work experience may be substituted for degree. Work in a human service environment a plus. Strong computer skills and attention to detail, including proficiency in Microsoft Office applications are required. Must have outstanding time management skills and be able to handle multiple tasks at a time. Must be responsible, have a positive attitude and compassion for older people. Excellent interpersonal and communication skills a must in this team-oriented position. Must have a valid driver's license and reliable transportation.
