Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Case Manager/Supported Employment and Education Specialist in Worcester.


The Supported Employment and Education Specialist will provide employment and educational support services such as information, training and advocacy in order to assist individuals to achieve their employment and/or educational goals. The Supported Employment and Education Specialist will provide a unique perspective and related skills to the program and perform tasks to assist individuals in increasing knowledge of and access to employment or education, and partner with individuals in the development of a plan to achieve personal growth.

CHL’s STEP (Screening and Treatment of Early Psychosis) program provides both patients and families with education, support and the highest quality medical care during the crucial early phase of psychotic disorders to improve long term outcomes. The program provides services to individuals between the ages of 18 and 40 who are experiencing early psychotic symptoms or who are experiencing psychotic prodrome, which are early signs that a psychotic illness might be about to happen. The program, located in Worcester, provides individual, group, family counseling, psychopharmacology, physical health monitoring and substance abuse screening and treatment.


  • Attend training related to employment counseling and complete certification as an employment specialist
  • Work as a member of an integrated team
  • Research and provide information about training opportunities and or classes related to employment interests and assist with enrollment and transportation when necessary
  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors
  • Grade C04 Case manager II-Requires a Bachelor’s degree; preferably in the field of Psychology or Human Services.
  • Provide transportation to interviews and job fairs and provide information about the employment services you can provide
  • Work closely with the clinician and the treatment team to create employment goals, objectives and interventions based on individual needs and preferences
  • Facilitate small group workshops when necessary to help individuals prepare for interviews, greeting employers, dressing for success and how to follow-up with employers
  • Current valid United States driver's license and ability to provide registered, inspected, and insured automobile for work related purposes including transporting Persons in a personal vehicle.
  • Provide support through every aspect of the employment process including, job research, completing applications and online assessments, creating cover letters, and practicing interview skills.

  • Problem solves issues related to employment to find opportunities and resources based on individual needs and interests
  • Must be certified or be eligible for the Mass Peer Specialist training and be certified within 6 months of employment.
  • Grade C02 Case Manager I-High school diploma/GED/HiSet required; Associates degree preferred.
  • Make regular contacts with employers in community to establish rapport and get job leads and keep track of employers you contact
  • Outreach to people in their homes, community, or programs where they reside to discuss their employment needs, strengths, skill level and possible barriers to employment
  • Previous/current participation in MH system as recipient of service and willingness to convey this experience as relevant,
  • Act as a liaison between those served and the employer to step in and problem solve issues that come up at work, provide follow along supports by checking in with the individual and the employer on a regular basis, and offer job coaching as needed
  • Conduct assessments to determine employment interests and skill level
  • Attend IAP and treatment team meetings to provide updates about progress
  • Connect individuals with Massachusetts Rehabilitation Commission, Beneplan and other vendors thought to assist with employment goals
