About The Role


Yuvo is hiring a Clinical Quality Program Manager to join our Population Health Team.


Yuvo Health is seeking a Clinical Quality Program Manager with experience working with quality health metrics to drive health outcome improvements. This position will be responsible for understanding all assigned market measures and ensure internal and external partners have a clear pathway for achieving and measuring success. The Clinical Quality Program manager will utilize quality improvement and project management experience to monitor progress made toward metric goals and appropriately identify and address any potential risks. Working closely alongside colleagues, this role will effectively and efficiently align improvement efforts to meet quality targets. The Clinical Quality Program Manager will report to the Director of Quality.


This is a fully remote position for employees based in the United States with required availability from 10:00 am EST to 3:00 pm EST.


Travel up to 25% to the New York City Metro area to work directly with your designated FQHC provider partners as needed.

What You’ll Do


  • Utilize standard project management tools and principles to define and manage project scope, monitor timelines and deliverables, and communicate and identify pathways to resolve risks and barriers.
  • Maintain responsibility for monitoring monthly and annual performance on quality targets across all health plans and health center partners.
  • Hire, train and supervisor employees which support the clinical quality department goals.
  • Develop and support improvement efforts to achieve quality targets.
  • Support the development and lead implementation of the quality plan.
  • Incorporate continuous process improvement into daily work using critical thinking and problem-solving skills to address opportunities for improvement while promoting engagement of stakeholders in solution development.
  • Perform chart reviews and audits to validate information and support supplemental files.
  • Oversee and maintain a portfolio of population health projects and initiatives to meet the aim of better care for individuals, better health for populations, smarter spending through improvement and care team well-being.
  • Use clinical, member, financial and administrative data to identify, develop and/or monitor actionable opportunities for improving health and healthcare outcomes.
  • Partner closely with health centers and Yuvo performance managers to drive positive outcomes.
  • Any other duties are reasonably required.


Requirements


WHAT YOU’LL BRING (REQUIRED)


  • Experience using Microsoft Excel for data organization and use of other data platforms
  • Experience working with various types of health organizations
  • Bachelor degree in a health related field
  • Familiarity with Value Based Care model
  • Quality Improvement and project management training/experience
  • Highly organized with ability to document progress in an absorbable way
  • Experience working with federal and state developed metrics to drive health organization improvement

Benefits


Base Salary Range: $100,000 - $120,000/year
