About The Role


Department: Certified Home Health Agency, Clinical Operations


Location(s) & Schedule Commitments: Webster NY. Primarily Monday through Friday 8:00AM- 4:30PM position with occasional need for off shift on call coverage required.


Position Summary: The Clinical Nurse Leader is responsible for designing, implementing and evaluating patient care by coordinating, delegating and supervising the clinical care delivered to patients receiving home care services within the agency’s geographic service area, while promoting Patient and Family Centered Care.


Responsibilities


Essential Responsibilities Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodations:


  • Support Clinical Team Managers by providing clinical nurse expertise, with a focus on complex clinical cases
  • Ensure nursing staff receive best practice skills education and professional development, from onboarding to independence as home care nurse, driving excellence in clinical practice standards and demonstrated clinical competency in the home care service environment
  • Serves as clinical resource for all nurses, putting evidence-based clinical practice into action with a focus on safety and quality while improving patient care delivered in patient homes

Qualifications


Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:


  • Education:
  • One year of home care experience preferred.
  • Experience:
  • Bachelors Degree in Nursing required
  • Masters Degree in Nursing preferred
  • Five years of experience as an RN, with 1-3 years demonstrated effective leadership experience preferred.


Licensure, Certification, and/or Credential Requirements


  • Clinical Nurse Leader certification preferred
  • Active New York State RN license
  • Valid NY State Drivers License and reliable transportation


Knowledge, Skill, & Ability Requirements


  • Outstanding interpersonal and communication skills both verbal and written.
  • Ability to stay current with and incorporate into clinical best practice: technological changes, fluctuating payment methods, new treatment modalities, and regulatory/legislative changes that impact care delivery in home care
  • Ability to work with patients, families, and staff from diverse backgrounds.
  • Clinical nursing expertise in adult population
  • Knowledge of research principles
