Job Summary:

The Clinic Care Coordinator Registered Nurse is responsible for coordinating and delivering comprehensive care for patients in a clinic setting. They will collaborate with healthcare providers and other team members to ensure efficient and effective care delivery. The primary focus of this role is to manage and organize patient care, advocate for patients, and provide education on treatment plans and preventive measures.

Responsibilities and Duties:

1. Coordinate all aspects of patient care in the clinic, including scheduling appointments, coordinating tests and procedures, and managing referrals.

2. Assess patient needs and develop individualized care plans based on their medical history, current conditions, and treatment goals.

3. Communicate with patients, families, and healthcare providers to ensure a smooth flow of information and continuity of care.

4. Provide patient education on medications, treatments, and self-care practices to enhance patient understanding and compliance.

5. Collaborate with interdisciplinary teams to establish comprehensive healthcare plans and ensure seamless care transitions.

6. Monitor and evaluate patient progress, making appropriate changes to care plans as needed.

7. Assist patients in navigating healthcare systems, providing resources, and ensuring they have access to necessary services.

8. Serve as an advocate for patients, addressing their concerns and helping them make informed decisions about their healthcare.

9. Maintain accurate and detailed documentation of patient assessment, care provided, and outcomes in compliance with regulatory requirements.

10. Participate in quality improvement initiatives to enhance the delivery and outcomes of patient care in the clinic.

Qualifications and Skills:

1. Valid and active Registered Nurse (RN) license in the state of employment.

2. Bachelor's degree in Nursing (BSN) or equivalent experience, Master's degree preferred.

3. Minimum of 2 years of clinical nursing experience in a hospital or clinic setting.

4. Strong understanding of healthcare systems, care coordination, and patient-centered care.

5. Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare providers.

6. Ability to work collaboratively in a team-based environment and effectively manage multiple priorities.

7. Demonstrated critical thinking and problem-solving abilities to address complex patient care needs.

8. Proficient computer skills, including electronic health records (EHR) and clinical documentation.

9. Understanding of regulatory requirements and quality improvement standards in healthcare.

10. Certified Care Coordinator (CCCTM) credential preferred.
