Job Description

PURPOSE OF YOUR JOB


The primary purpose of the CNA job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors.


DELEGATION OF AUTHORITY


As a Certified Nurse Aide, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.


I. ADMINISTRATIVE FUNCTIONS

A. Record all entries on Caretracker, flow sheets, notes, charts, etc., in an informative and descriptive manner.

B. Use only authorized abbreviations established by this facility when recording information.

C. Report all changes in the resident’s condition to the Charge Nurse/Nurse Manager as soon as practical.

D. Report all accidents and incidents you observe on the shift that they occur.


II. ADMISSION, TRANSFER AND DISCHARGE FUNCTIONS

A. Make resident comfortable (i.e. put to bed, help with toileting, etc.)

B. Transport residents to and from rooms as necessary.

C. Assist with loading/unloading residents from vehicles as necessary.


III. PERSONNEL FUNCTIONS

A. Perform all assigned tasks in accordance with our established policies and procedures and as instructed by your supervisors.

B. Follow work assignments, and/or work schedules in completing and performing your assigned tasks.

C. Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents.

D. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.

E. Meet with your shift’s nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services.

F. Report all complaints and grievances made by the residents, visitors or family members.

G. Notify the facility when you will be late or absent from work. (Note: failure to comply with this requirement may be grounds for dismissal.)

H. Report occupational exposures to blood, body fluids, infectious materials and hazardous chemicals to your supervisor.


IV. PERSONAL NURSING CARE FUNCTIONS

A. Participate in and receive the nursing report upon reporting for duty.

B. Follow established policies concerning exposures to blood/body fluids.

C. Assist residents with daily dental and mouth care (i.e. brushing teeth/dentures, oral hygiene, special mouth care, etc.)

D. Assist residents with bath functions (i.e. bedbath, tub or shower bath, etc.) as directed.

E. Assist residents with dressing/undressing as necessary.

F. Assist residents with hair care functions ( i.e. combing, brushing, shampooing, etc.)

G. Assist resident with nail care (i.e. clipping, trimming, cleaning of finger and toenails). Note: This does not include diabetic residents.

H. Shave male residents.

I. Keep hair on female residents clean shaven (i.e. facial hair, under arms, on legs, etc.) as instructed.

J. Keep residents dry (i.e. change gown, linen, etc.) when it becomes wet or soiled.

K. Change bed linens. Keep linens tight to avoid wrinkles from forming under the resident.

L. Make beds (occupied and unoccupied).

M. Put extra covers on beds as requested.

N. Position bedfast residents in correct and comfortable position.

O. Assist resident with bowel and bladder functions (i.e take to bathroom, offer bedpan/urinal, portable commode, etc.)

P. Maintain intake and output records as instructed.

Q. Keep incontinent residents clean and dry.

R. Check and report bowel movements and character of stools as instructed.

S. Collect specimens as instructed (i.e urine, sputum, stools, etc.).

T. Assist resident in preparing for medical tests (i.e. lab work, x-ray, therapy, dental, etc.).

U. Assist residents in preparing for activity and social programs (i.e. church services, parties, visitors, etc.)

V. Assist in transporting residents to/from appointments, activity and social programs, etc. as necessary.

W. Assist with lifting, turning, moving, positioning, and transporting residents into and out of bed, chairs, bathtubs, wheelchairs, lifts, etc.

X. Perform restorative and rehabilitative procedures as instructed.

Y. Provide eye and ear care, cleaning eyeglasses/hearing aids, etc. as instructed.

Z. Assist in preparing resident for physical examination.

AA. Weigh and measure residents as instructed.

BB. Measure and record vital signs as instructed.

CC. Answer resident calls promptly.

DD. Ensure that residents who are unable to call for help are checked frequently.

EE. Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.

FF. Assist with the care of the dying resident.

GG. Provide post-mortem care as instructed.


V. SPECIAL NURSING CARE FUNCTIONS

A. Observe and report the presence of pressure areas and skin breakdown to prevent decubitus ulcers (bedsores).

B. Report injuries of an unknown source, including skin tears.

C. Provide daily indwelling catheter care.

D. Provide daily perineal care.

E. Assist with the application of slings, elastic bandages, binders, Ted hose, etc..

F. Give tepid sponge baths.

G. Provide daily Range of Motion Exercises. Record data as instructed.

H. Turn bedfast residents at least every two (2) hours.

I. Observe disoriented and comatose residents. Record and report data as instructed.

J. Turn all medications found in the resident’s room/possession over to the charge nurse or Director of Nursing.

K. Watch for and report any change in room temperature, ventilation, lighting, etc.


VI. FOOD SERVICE FUNCTIONS

A. Prepare residents for meals (i.e. take to bathroom, wash hands, comb hair, raise bed, position tables, place bibs, take to/from dining room, etc.).

B. Helps residents complete their menu. Help residents through buffet line as necessary. Assist with feeding as indicated (i.e. informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.).

C. Record the resident’s food/fluid intake as indicated. Report changes in the

resident’s eating habits.

D. Keep residents’ water pitchers clean and within easy reach of the residents.

E. Serve between meal and bedtime snacks.

F. Perform after meal care (i.e. remove trays, clean resident’s hands, face, clothing, take to bathroom, brush teeth, clean dentures, etc.).

G. Check rooms for food (i.e. food in proper containers, unauthorized food items).


VII. STAFF DEVELOPMENT

A. Attend and participate in scheduled training and educational classes to maintain current certification as a CNA.

B Attend and participate in scheduled orientation programs and activities.

C. Attend and participate in annual Safety and Accident Prevention and Infection Control in-service programs for hazard communication, TB management, and blood borne pathogens standards.


VIII. SAFETY AND SANITATION

A. Participate in appropriate in-service training programs prior to performing tasks that involve potential exposure to blood/body fluids.

B. Wash hands before and after performing any service for the resident.

C. Keep the nurses’ call system within easy reach of the resident.

D. Immediately notify the charge nurse/DON of any resident leaving or missing from the facility.

E. Follow established safety precautions in the performance of all duties.

F. Keep residents personal possessions off the floor and properly stored.

G. Keep floors dry. Clean spills per protocol.

H. Keep excess supplies and equipment off the floor. Store in designated areas.

I. Wash wheelchairs, walkers, etc., as instructed.

J. Clean, disinfect and return all resident care equipment to its designated storage area after each use.

K. Perform routine housekeeping duties (i.e. clean bed rails, over bed tables, night stand,, etc.), that relate to nursing care procedures.

L. Before leaving work areas for breaks, or at the end of the shift, store all tools, equipment, and supplies.

M. Report all hazardous conditions and equipment to the charge nurse or Director of Nursing immediately.

N. Report all safety violations.

O. Follow established smoking regulations - report all violations.

P. Report any communicable or infectious disease to the Infection Control Coordinator or the Director of Nursing.

Q. Follow established standard isolation precautions and procedures.

R. Wash hands before entering and after leaving an isolation area.

S. Follow established procedures in the use and disposal of personal protective equipment.

T. Wear and/or use safety equipment and supplies (e.g. back brace, gait belt, mechanical lifts, etc.,) when lifting or moving residents.

U. Report missing/illegible labels and SDS sheets to your supervisor.


IX. EQUIPMENT AND SUPPLY FUNCTIONS

A. Use only the equipment you have been trained to use.

B. Operate all equipment in a safe manner.

C. Use only the equipment and supplies necessary to do the job. Do not be wasteful.

D. Report defective equipment to the Charge Nurse/Director of Nursing.

E. Inform the charge nurse of your equipment/supply needs.


X. CARE PLAN FUNCTIONS

A. Review care plans to determine if changes in the resident’s daily care routine have been made on the care plan.

B. Inform the Care Plan Coordinator or Charge Nurse of any changes in the resident’s condition so that appropriate information can be entered on the resident’s care plan.

C. Ensure that your notes reflect that the care plan is being followed.


XI. RESIDENT RIGHTS

A. Maintain the confidentiality of all resident care information.

B. Ensure that you treat all residents fairly, and with kindness, dignity and respect.

C. Provide privacy for the resident during the provision of personal care by closing resident’s room door, room window drapes and/or bathroom curtain.

D. KNOCK before entering a resident’s room.

E. Report all grievances and complaints made by the resident to the charge nurse or Director of Nursing.

F. Report all allegations of resident abuse and/or misappropriation of resident property per Bethesda Home’s Abuse, Neglect and Exploitation Policy.

G. Honor the resident’s refusal of treatment request. Report such requests to your supervisor.


WORKING CONDITIONS


  • Works throughout the facility (i.e. nurses’ station, resident rooms, bathing areas, etc.).
  • Is subject to frequent interruptions.
  • Communicates with nursing personnel and other department personnel.
  • Works beyond normal working hours, on weekends and holidays, and in other positions temporarily, when necessary.
  • Is subject to call-back during emergency conditions (e.g. severe weather, evacuations, post-disaster, etc.)
  • Attends and participates in continuing educational programs.
  • Is subject to injury from falls, burns from equipment, odors, etc., throughout the work day, as well as to reactions from dust, disinfectants and other air contaminants.
  • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB, AIDS and the Hepatitis B viruses.
  • May be subject to the handling of and exposure to hazardous chemicals.

EXPERIENCE


Must be currently certified as a Nurse Aide in accordance with Kansas law.


SPECIFIC REQUIREMENTS


  • Must be able to read, write, speak and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability and willingness to work harmoniously with other personnel.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into exiting nursing practices.
  • Must be able to relate information concerning a resident’s condition.
  • Must not pose a direct threat to the health and safety of other individuals in the workplace.

  • Must meet the general health requirements set forth by the policies of this facility.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset and, at times, hostile people within the facility.
  • Must be able to push, pull, transfer 50 pounds unassisted

Must be able to push, pull, transfer 50 - 100 pounds with the assistance of another staff member.

  • Must use mechanical lift when lifting - transferring any non-weight bearing individual weighing more than 100 pounds.
  • May be necessary to assist in the evacuation of residents during emergency situations.
