Position Summary

The position of Case Worker I is intended to provide client-centered HIV case management services and thorough bio-psychosocial assessments for individuals in coordination with the multidisciplinary healthcare team. The position will create and implement multidisciplinary care plans to include identification of educational needs and community resource referral and coordination; provide crisis intervention and supportive counseling to assist the client/family in coping with illness, disclosure of diagnosis, confidentiality issues, familial grieving, and death/dying; and coordinate access to resources in accordance with all applicable local, state, and federal guidelines and procedures. The Case Worker I will abide by the same strict standard of patient confidentiality that applies to all Washington University staff. The Case Worker I will work under the under the direction of senior staff.

Primary Duties and Responsibilities

The Case Worker I will work collaboratively with the multidisciplinary healthcare team to provide case management services to clients as assigned, including the following expectations:

  • Independently manage caseload by providing regular contact with enrolled clients and documenting all contacts and service coordination activities in client records as required by program guidelines.
  • Maintain current knowledge of medical and psychosocial developments concerning HIV/AIDS and related conditions, and routinely interact and network with ASOs, providers, clinicians, and others involved in HIV/AIDS service delivery.
  • Perform indirect, non-clinical tasks related to the job, including compilation of data, participation in required training, and attendance at staff meetings and community meetings as assigned.
  • Adhere to all applicable state laws and the department’s rules and regulations regarding HIV/AIDS programs.
  • Function as part of a multidisciplinary HIV Specialty Health Care Team, and meet as scheduled in case conferences to ensure coordinated delivery of care to patient and family.
  • Conduct a comprehensive bio-psychosocial culturally-sensitive assessment with the client and family/support network as appropriate. Synthesize program goals and assessed needs to create an individualized plan of care for each client.
  • Collaborate with case managers from other regions as needed when a patient resides outside of program service boundaries.
  • Arrange and coordinate service referrals for the client/family as needed; provide ongoing monitoring of the activities implemented to evaluate progress on the plan of care. Refer clients/families to public benefits programs as eligible.
  • Educate clients/families on HIV/AIDS, medication adherence and safer sex.
  • Educate clients/families about the basic medical and psychosocial aspects of HIV/AIDS in order to optimize adherence to care.

Other Functions

  • Work collaboratively within Adult ID Clinic multidisciplinary team and the Project ARK social services team.
  • Attend monthly Ryan White community-wide case management meetings.
  • Assist with other client support activities as deemed appropriate.

Preferred Qualifications

  • Computer skills.
  • Master’s of Social Work.
  • Provision of case management assistance and support.
  • Knowledge of community resources.

Required Qualifications

Equivalent of a bachelor’s degree in a health related field, such as social work, counseling, sociology, or psychology. Applicants with a bachelor’s degrees in other fields may be considered with sufficient case management experience.

Salary Range

$19.77 - $29.64 / Hourly

The salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
