Summary
Job Summary
Carries out psychosocial assessments, while providing psychosocial support/advocacy to palliative care patients and families as part of the palliative care consult team. Provides clinical support to the palliative care team and hospital staff regarding psychosocial patient/family needs.
Job Requirements
Education
Required: Master's Degree in Social Work
Experience
Required: Minimum 2 years clinical experience
Preferred: Prior experience in hospice/palliative care setting
Skills
Good communication skills – Demonstrates the use of effective verbal, non-verbal, and written communication with patients, families and members of the healthcare team in order to therapeutically address and accurately convey the palliative care needs of patients and families. Communicates compassionately about end-of-life issues.
Strong analytical and problem solving skills. Demonstrates good critical thinking, analysis and clinical judgment through the social work process to address the physical, emotional, psychosocial and spiritual needs of the patients and families.
Comprehensive assessment skills. Ability to write effective documentation for processes and procedures. Ability to make routine decisions in accordance with departmental policies and procedures.
Learn and appropriately utilize all required computer applications, including but not limited to E-mail, Windows-based or Microsoft programs
Exhibits initiative, responsibility and flexibility. Ability to maintain organization in a changing environment.
Incorporates ethical principles and professional standards in the care of patients and families. Recognizes one’s own attitudes, feelings, values, and expectations about death and the individual, cultural, and spiritual diversity existing in these beliefs and customs.
Has broad understanding of and experience with patients who are suffering from advanced/chronic illness and/or end of life.
