Job Description

JOB OPENING:THE KIND PLACE

We are looking for a Case Manager to join our team. In this rewarding position, you will provide home and community supports to individuals, who have had a mild to moderate brain injury, to be more successful in their independent living. We currently have need for a part time employee in Springfield. 20-32 hours per week.We offer a competitive hourly wage and paid mileage.

The successful candidate will be responsible for assessing client needs, following individualized plans, coordinating with service providers, and advocating for/with clients in order to ensure their needs are met. The ideal candidate will have a kind heart, excellent problem solving and communication skills, as well as a passion for helping others. The following skills are a plus: Knowledge of community resources, experience working with individuals who have had a traumatic brain injury, bachelor's degree.

Responsibilities:

  • Develop and maintain relationships with other service providers to ensure continuity of care.
  • Participate in case reviews and team meetings.
  • Monitor client progress and adjust service plans as needed.
  • Provide referrals to appropriate resources, such as housing, employment, or medical services.
  • Maintain accurate records of client services and progress.
  • Provide crisis intervention and support services to clients.
  • Assess client needs and be part of the team that develops appropriate service plans.

Requirements:

·Valid drivers license, clean driver record, auto insurance, willingness to transport clients serviced in your own vehicle.

·Mature composure

·Highschool diploma (bachelor’s preferred)

·Clean criminal history

·Kind and patient heart

Job Types: Part-timepotentially moving to FULL-TIME Salary: $13.00 - $18.00 per hour
