Care Coordinator - Teen Mental Health

Responsibilities The purpose of this position is to provide a wide range of community support and coordination of services to clients. The Care Coordinator will also link, refer, and coordinate people with available community resources. They also assist with co-facilitation of group services when appropriate.
Participates in the development and coordination of the treatment plan
Coordinates crisis intervention services
Care coordination with other community partners, referral agencies, and invested parties
Assist with co-facilitation of groups including facilitation of activities, coordination of transportation or patient food needs
Serves as an advocate for clients and communicates with significant supports of client
Confers with supervisor and other team members regarding client needs
Required Skills Bachelor’s degree in social work or a related field or High School diploma with 3+ years of experience in a social service field. QMHS Certified preferred.
Knowledge of community resources. Excellent written and oral skills.
