We have an opening for a Full-Time Care Coordinator LPN to join the amazing team at our Employer Health Center located in Canton, MS! Schedule: Monday - Friday, 7:30AM - 3:30PM
PURPOSE OF THE POSITION
The Care Coordinator is responsible for reviewing a member’s health history in Epic to identify gaps (missed visits/labs, prescription/refills, member questions, outside visits including ED and Urgent care). The Care Coordinator will conduct member outreach to schedule visits according to provider request or member need. Additionally, the Care Coordinator is responsible for supporting the Care Navigator by following up after appointments to ensure complete documentation and following up with the member for feedback and next steps.
ESSENTIAL JOB FUNCTIONS
- Provides written status updates on a regular basis as requested and verifies documentation of all such activities in member chart.
- Coordinates assessment, treatment plans, and return to work processes with the employee, providers, and employer.
- Counsels clients, assisting them to identify and resolve problems and make effective use of resources.
- Works with community specialists and members to schedule appointments and ensure bi-directional notes transfer.
- May be asked to obtain and record patient's vital signs and weight.
- Interacts with Premise providers to understand referral needs.
- Assesses and evaluates cases and implements treatment and case management plans.
- Maintains collaborative working relationships with other community providers to develop specialized services as required.
- Monitoring of the plan addressing at risk areas with documented follow up.
- Follows up with member post-specialist visit, ensure member and specialist notes return to Premise, and coordinate ongoing care needs as appropriate.
- Participates in case conferences to ensure that each member’s care team is aligned with a shared plan of care and up-to-date information on each member.
- All other duties as assigned.
- Proactively builds relationships with community specialists, alongside other health center team members, to educate them on Premise and our expectations, facilitate streamlined referral access and information sharing.
- Develops an understanding of where member populations live and work, as well as high-value providers and facilities in key geographies.
- Interacts with members via multiple modalities (phone, email, in-person, video, etc) to identify appropriate referral target(s).
- Provides communication to staff on specific cases.
- Remains in close coordination with central Member Health Management team as we evolve our Member Health Management strategy, leveraging new tools and processes.
- Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required.
- Identifies, facilitates and secures access to needed medical and mental healthcare services and appointments, social service benefits and community resources for the client’s member population and dependents.
- Complete accurate and up to date documentation; conduct daily follow ups to address any issues, review the previous day’s activities, and to discuss any client issues and or/new insight regarding a member.
- Maintain a directory of provider services including medical, behavioral health, social support and other services as needed to support referral and coordination activities. Identify valuable resources to share with the rest of the team to the benefit of all members.
- Complete an in-depth initial needs assessment; subsequently overseeing a member-centered plan to address at risk areas.
- Develops an understanding of Epic's referral and scheduling capabilities, and available tools.
BASIC QUALIFICATIONS
Education:
- Graduate of National League for Nursing accredited program, Associate's degree (A.S.) and/or equivalent from two-year college or technical school.
- Licensed as a Practical or Vocational Nurse
- Current hands-on Certification in Basic Life Support is required, Advanced Cardiac Life Support may also be required based on scope of contract services.
Experience:
- At least three years of clinical experience as a LPN/LVN in an ambulatory care, occupational health, or emergency department setting.
Knowledge and Skills:
- Excellent problem-solving skills and ability to build positive relationships
- Be able to work in and create a positive attitude environment, manage caseload, complete task/duties and address conflicts as they arise.
- Be able to tolerate the ambiguity during transition time and take on other assignment as needed.
- Communicate effectively with supervisory personnel in a cooperative and supportive manner.
- Excellent verbal and written communication skills in a customer service environment
- Excellent time management, organization, research, analytical, negotiation and interpersonal skills
