Job Description

The Care Coordinator is responsible for assisting acute care inpatients with care transitions throughout the healthcare continuum. The Care Coordinator is responsible for ensuring quality patient outcomes, monitoring resource utilization, and working through barriers to care delivery via interdisciplinary collaboration, communication, and coordination. This includes monitoring and coordinating treatment plans with patients in order to educate them about their diagnosis(es) and connect them with various healthcare teams and providers, as needed.
  • Works in collaboration with the attending physician to identify an appropriate length of stay while monitoring resource utilization
  • Maintains appropriate documentation
  • Works with patients, families, and caregivers to ensure patient needs are met, and a timely and appropriate discharge plan is implemented.
  • Assesses for and develops an individualized discharge plan.
  • Must have excellent written/verbal communication and interpersonal skills
  • Must have the ability to work as part of a multidisciplinary team and with patients of various ages and cultural backgrounds
Job Requirements
Applicable Experience:
1 year
Basic Life Support - American Heart Association or Red Cross, Registered Nurse - Various
Bachelor's Degree - Science in Nursing
