Job Summary

Baptist Health is lookin g to add a Cardiac Rehab Coordinator to our Cardiac Rehab Unit at Baptist Clay in Fleming Island, FL. This is a Part-Time, day shift opportunity.

As a Cardiac Rehab Coordinator , you will be responsible for:

• Goal setting, education, exercise prescription/progression, and discharge planning.
• Supervises Cardiac and Pulmonary staff in the day to day operations.
• Specific duties include new patient assessment, on-going patient assessment, and development of Individual Treatment Plan.
• Completing appropriate documentation/charges and communicates to physician as needed.
• Clinical patient treatment for Cardiac and Pulmonary Rehab patients involved in Phase I-III.

Education Required

Bachelor's Degree

Education Preferred

Master's Degree

Experience

• Pulmonary Rehabilitation Experience
• 3-5 Years Clinical Cardiac Experience

License and Certification

• Certified Cardiac Rehab Professional (CCRP) Required Or
• Certified Clinical Exercise Physiologist (CEP) - ACSM Required Or
• Basic Cardiac Life Support (BCLS) Required
