Job Description

Spectrum Health Services is in search of a dynamic Behavioral Health Consultant to join its team of exceptional providers. The ideal candidate will be a Licensed Clinical Social Worker, Licensed Psychologist, or Licensed Professional Counselor in the state of Pennsylvania with no restrictions to practice within the scope of their license and possess at least 3-5 years post Master's experience.


In this role, you will provide behavioral health consultation as part of a team-based model of care delivery to primary care patients. You will partner to improve the psycho-social functioning and the self-management of chronic illness in children, adolescents, adults, and families.


Essential Functions:

  • Work collaboratively across disciplines and administrative functions within the organization and the surrounding community to identify and help manage 'at risk' 'chronically ill,' and 'frequent attenders.'
  • Teach prevention and techniques to self-manage symptoms and chronic illness.
  • Facilitate support groups e.g. chronic disease-self management, pain management, smoking cessation.
  • Work in a 'readiness to change' (Stages of Change) model versus a long term therapeutic model of care.
  • Represent the heath agency at external events and meetings.
  • Develop educational, training materials and conducts sessions or disseminates information to care team on recent advances in the field.
  • Apply knowledge of and experience with a broad repertoire of psychotherapeutic skills from a variety of theoretical perspectives (e.g. Motivational Interviewing, Solutions Focused Tx,, Mindfulness techniques, Acceptance & Commitment Tx, and CBT).
  • Help patients, families, and caregivers build skills to improve coping, distress tolerance, and stress reduction.
  • Provide diagnostic assessments and effective triage of patients in need of referrals to specialty mental health care
  • Develop proficiency in fundamental medical terminology and education as it relates to chronic illness e.g. diabetes, hypertension, COPD/asthma, pain disorders
  • Develop collaborative relationships with external medical, behavioral, and social service community partners.
  • Provide professional expertise and consultation to Primary Care Providers and the care team within a population-based care framework.
  • Participate in Quality Improvement/Quality Assurance activities as appropriate.
  • Perform strengths-based assessment and brief behavioral treatment for a wide range of psychological and substance abuse disorders. Refer to specialty clinics as appropriate.

Required Experience:

  • Desire to work with diverse under-served populations.
  • Comfort and flexibility in working in a fast-paced health environment.
  • Experience with interdisciplinary team-based care preferred.
  • Familiarity with Medical Assisted Treatment (MAT).
  • Licensed Clinical Social Worker (LCSW) or Licensed Psychologist: PsyD or PhD, or Licensed Professional Counselor (LPC)
  • Medical literacy and knowledge of psychopharmacology desired.
  • Experience in substance abuse counseling.

· Medical/prescription/vision/dental – SHS pays 85% of the premium for employee and 75% of premium for dependents.
