Behavioral Health Primary Health Coordinator

If so, we’d love to connect with you! We are Mental Health Cooperative (MHC), and we are looking for a Primary Health Coordinator to join our team! You will help within the team environment agency-wide, and in partnership with physical healthcare providers and specialists. This role assists with consumers to assist with closing gaps in care to manage mental and physical health. You will provide education, support, and guidance to teams regarding gaps in care.

To Join Our Team, You’ll Need

• Collaborative spirit working with team members to meet consumers’ needs
• Bachelor’s Degree in a Behavior Health Field
• Ability to multi-task consumers’ needs
• Compassionate response to consumers’ needs while maintaining healthy boundaries
• To be an active listener who can identify obstacles, and conceive solutions for the development of individualized care
• Ability to transport consumers to appointments in a safe and reliable manner

We’re Especially Looking For Someone With Social Work Experience Within a Mental Health Clinic, School, Agency, Or Hospital Environment Who Is

• Flexible to changing priorities and needs, especially during crisis
• Dependable to both consumers and team members
