About the job:
Schedule: Monday-Friday 8:00 AM-4:30 PM
Pay: $26-$30/HR
The Assistant Home Care Coordinator supervises PCAs and CNAs in the home and day center environment to ensure adequate staffing is obtained to meet the needs of participants. Supervises all direct staff in conformance with policies, procedures, and standards and all governing state and federal regulations.
Required Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
- Must have a valid driver’s license, proof of insurance and have means of transportation.
- LPN, and Cardio Pulmonary Resuscitation (CPR) certified.
- Must have high school diploma or equivalent certification
- Must have medical clearance for communicable diseases and up-to-date immunization before having direct participant contact.
- Two years of working with older adults in a healthcare setting with demonstrated successful supervisor experience preferred.
Preferred Skills:
- Working knowledge of the PACE regulations and Michigan regulations
Essentials:
- Complete supervisory visits for all CNA/PCAs every 90 days
- Complete LOCDs for existing participants
- Complete all competencies for CNA/PCAs – new and annual
- Directs, supervises and evaluates the performance of the Personal Care Service Supervisor (PCSS)
- Rotate on-call scheduling responsibilities with PCSS’
- In conjunction with PCSS will complete first HC visit upon discharge from SAR to ensure appropriateness of care plan and staffing
- In conjunction with PCSS and HCC will observe visits of higher acuity/challenging ppt.’s
- Complete care hour reassessments for ppt.’s at ALFs (as asked for by facility)
- Back-up to HCC for routine home-care assessments
