Job Description

Job Summary

Monitors and provides support to patients and facilitate various recovery-oriented activities.

Job Duties

  • Communicates significant events via communication log, patient charts and verbally to key staff.
  • Completes initial family contacts and provide information on family services and document appropriately and facilitates sign-up for Family Day Program and monitor family visitations.
  • Provides support to patients during evenings and weekends, and when needed, help to de-escalate any emotional reactions patients might have.
  • Collects appropriate data on bio-psycho-socials.
  • Monitors patients during free time and their preparation for bedtimes and conduct bed and room checks.
  • Provides new patients with general orientation to the facility, including but not limited to, introducing new patient, initial assignments, buddy system and introduction to the facility rules and regulations.
  • Facilitates or assists with adjunct groups/meetings and performs all associated duties, including documentation.

Education

High School Diploma or Equivalent (GED)- (Required), Bachelor's Degree-Social Sciences (Preferred)

Experience

Minimum of 1 year-Related work experience (Preferred)
