Responsibilities Of The Community Support Specialist
Provide wraparound assessment for service planning
Assist in and monitor treatment plan development and complete necessary quarterly reviews
Maintain all assigned client medical records to program standards and complete documentation in a timely manner (within 72 hours)
Provide contact with families at minimum of twice per month based upon client/family needs, one at least face-to-face in the home

Overview Youth Support Specialists are responsible for providing support and guidance to young people in a variety of settings. They work with young people to help them develop the skills and knowledge needed to make positive life choices. They also provide guidance and support to parents and other adults who are responsible for the care and development of young people. Detailed Job Description Youth Support Specialists are responsible for providing support and guidance to young people in a variety of settings. They work with young people to help them develop the skills and knowledge needed to make positive life choices. They also provide guidance and support to parents and other adults who are responsible for the care and development of young people. They may also provide counseling and mentoring services to young people and their families. Youth Support Specialists are responsible for assessing the needs of young people and developing individualized plans to meet those needs. They provide guidance and support to young people in areas such as education, employment, health, and social development. They also provide referrals to appropriate services and resources. Youth Support Specialists are responsible for monitoring the progress of young people and providing feedback to parents and other adults. They may also provide crisis intervention services when needed. Job Skills Required

Job Qualifications
• Bachelor’s degree in social work, psychology, or a related field
• Master’s degree in social work, psychology, or a related field preferred
• Certification or licensure in social work, psychology, or a related field preferred
• Previous experience working with young people in a professional setting preferred
Job Knowledge
• Knowledge of child and adolescent development
• Knowledge of community resources and services
• Knowledge of crisis intervention techniques
• Knowledge of relevant laws and regulations
• Knowledge of relevant ethical and professional standards
Job Experience
• Previous experience working with young people in a professional setting preferred
• Previous experience working with diverse populations preferred
