Detailed Job Description:

Willcare HCBS is looking for a qualified individual to provide home and community-based services to individuals with disabilities and other special needs. The successful candidate will be responsible for providing personal care, respite care, and case management services to clients in their own homes and communities. The individual will also be responsible for developing and implementing individualized service plans, monitoring client progress, and providing support and guidance to clients and their families.
