Is personally responsible and held accountable for work hours and time management as coordinated with the Team Leader.
Works in partnership with parents who are treated as experts concerning their families.
Helps to conduct strength/needs assessment; assists with and participates in interdisciplinary team meetings.
Responsible for submitting/entering outcomes/incident data into identified database or providing this to person responsible for entering the information.
Coordinates care as needed, internally and with outside agencies; Coordinates care with system partners.
Keeps START Coordinator and supervisors informed of significant treatment or care coordination issues.
